If you have patients who meet Acute Rehabilitation criteria (ages 3-21) or patients who require subacute services (ages 0 to 21), please call our Case Management Department, 909-796-6915, extension 130 or 131. Melinda or Maria will be happy to help you through our admission process. They would also be able to arrange for a visit from our team to explain our services and learn how we can best serve your hospital’s and patient’s needs.
